What is an Accredited Exercise Physiologist?
As university trained allied health professionals, Accredited Exercise Physiologists (AEP) are equipped with specialist knowledge and skills enabling them to assist individuals initiate and maintain lifestyle and behaviour modification programs.

With particular emphasis on exercise delivery aimed at the prevention and management of chronic disease and injury, AEP’s can assist clients with complex conditions including but not limited to cardiovascular disease, diabetes, osteoporosis, depression, cancer, arthritis, COPD and many more. Chronic Disease Management Services

As AEP’s are recognised allied health professionals, patients may have access to Medicare, DVA and private health rebates.

 

Medicare Services

  • Item 10953 – Allied Health Services under Medicare
    Eligible patients will have access to a maximum of 5 services per calendar year and must have either an EPC Program or GP Management Plan and Team Care Arrangement in place and be referred by their doctor. Services can be claimed through either Medicare or private health insurance (patients must choose one option only).

  • Items 81110 & 81115 – Allied Health Group Services under Medicare for patients with Type 2 Diabetes
    • Items 81110 - This initial assessment is a one-on-one consultation designed to assess a patients’ suitability for allied health group services. To be eligible patients must have Type 2 Diabetes, have a relevant care plan in place and be referred by their doctor
    • Item 81115 – Eligible patients will have access to a maximum of 8 one-hour group sessions (2-12 people) per calendar year. (These services are in addition to the 5 individual services for patients with a chronic medical condition and complex care needs). Non-Medicare full-fee paying patients are also welcome to attend.
  • Item 81513 – Aboriginal & Torres Strait Islander Follow-Up Services

    People of Aboriginal or Torres Strait Islander descent who have had a health check may be eligible for up to 5 individual services per calendar year. (These services are in addition to the 5 individual services for patients with a chronic medical condition and complex care needs).

Lifestyle Modification Programs

 

Reset Your Life

Reset Your Life is a government subsidised lifestyle modification program targeted at those patients who have been identified as being at high risk for developing Type 2 Diabetes. Through a series of group education classes, patients will be provided with the necessary knowledge, skills and support to begin making lifestyle changes to prevent or delay the onset of Type 2 Diabetes.

 

To be eligible patients must:

  • be aged between 40 and 49 years (15 - 54 years for Aboriginal or Torres Strait Islander people)
  • be determined as 'high risk' using the AUSDRISK screening tool
  • have had a diagnosis of Type 2 Diabetes excluded
  • have had a health assessment (items 701, 703, 705, 707 or 715 for Aboriginal or Torres Strait Islander people)

The 6-month course will focus on:

  • the risks of diabetes and their relationship to lifestyle factors
  • the importance of regular diabetes screening
  • nutrition advice and education
  • physical activity advice
  • preventing cardiovascular disease
  • motivation and positive thinking
  • stress management
  • behavioural strategies to support the adoption and maintenance of lifestyle changes
  • smoking cessation and alcohol reduction advice or referral
  • information about community resources relevant to sustaining lifestyle changes

Heartmoves

What is Heartmoves?

Heartmoves is a  low-to-moderate intensity physical activity program developed by the National Heart Foundation. It is suitable for older adults, people who haven't done any exercise for awhile and those with or at risk of stable chronic health conditions. The empasis is on increasing activity levels with patients encouraged to exercise at their own pace.

Heartmoves sessions include:

  • warm-up and cool-down
  • stretching
  • aerobic and resistance training
  • balance, coordination and flexibility

What benefits could my patients experience?

Heartmoves can help:

  • lower blood pressure
  • improve cholesterol control
  • manage weight
  • improve wellbeing and quality of life
  • manage diabetes
  • improve balance and flexibility
  • improve sleep
  • improve bone mineral density
  • prevent falls and injuries from falls


Who can participate?

Everyone is welcome to attend Heartmoves. It is designed to be safe for people with (or at risk of) stable chronic health conditions including those with heart disease, diabetes, obesity, arthritis, high blood pressure, high cholesterol, osteoporosis, lung conditions and cancer. 

Joining a Heartmoves class may help your patients:

  • increase their physical activity levels
  • maintain a healthy lifestyle
  • continue their exercise after attending a rehabilitation program
  • have fun and meet new people
