Lifestyle Modification Programs
Reset Your Life
Reset Your Life is a government subsidised lifestyle modification program targeted at those patients who have been identified as being at high risk for developing Type 2 Diabetes. Through a series of group education classes, patients will be provided with the necessary knowledge, skills and support to begin making lifestyle changes to prevent or delay the onset of Type 2 Diabetes.
To be eligible patients must:
- be aged between 40 and 49 years (15 - 54 years for Aboriginal or Torres Strait Islander people)
- be determined as 'high risk' using the AUSDRISK screening tool
- have had a diagnosis of Type 2 Diabetes excluded
- have had a health assessment (items 701, 703, 705, 707 or 715 for Aboriginal or Torres Strait Islander people)
The 6-month course will focus on:
- the risks of diabetes and their relationship to lifestyle factors
- the importance of regular diabetes screening
- nutrition advice and education
- physical activity advice
- preventing cardiovascular disease
- motivation and positive thinking
- stress management
- behavioural strategies to support the adoption and maintenance of lifestyle changes
- smoking cessation and alcohol reduction advice or referral
- information about community resources relevant to sustaining lifestyle changes
